We are currently checking that all our patients contact details are up-to-date.

Therefore, we would be grateful if you could ask Reception for a contact details slip, even if you think your details are correct, so we can verify these on your patient record.

It is important that your contact details are correct so that we are able to get hold of you when needed. If you consent to having texts/emails sent to you then this gives us the option to use these pathways instead of having to contact you by post or telephone.
